New Policy and Privacy Documents Posted June 29, 2015

The new Consent Form includes sharing of veteran data with the Veteran's Administration (VA).  Please start using this new consent form immediately. There is also a new question on the SubCook Comprehensive Entry Assessment where you will need to indicate whether the veteran client has agreed to share data with the VA (note that veterans do have the option of not sharing with the VA). The new consent form also reflects sharing of data with our partners in Northeast Illinois, which now includes DuPage County

Current Veteran Clients--it is of the utmost importance to share veterans' client data with the VA to optimize housing options for these clients.  Please contact any veteran clients currently in your projects and have them complete this new consent form and then indicate their consent on the SubCook Comprehensive Entry Assessment.  We have created a NEW ART report that will produce a list of all veterans currently entered in your projects.  The report is located in the "03 Suburban Cook (Common Share)" ART folder and is called "002 NEW Veteran Client List".

The Notice of Privacy Practices has been modified to reflect the sharing of data with HMIS partners in Northeast Illinois and with the Veteran's Administration. 

The HMIS Policies and Procedures Manual has been revised to reflect the new HMIS Data standards that took effect October 1, 2014, allows for verbal consent to share data collected by telephone intake operations (protected personal information still requires written consent to share), and reflects our new partnership with DuPage County.  It replaces the previous version of the manual and takes effect immediately.
