Health Service Model Project for PSH Providers

To develop a better understanding of which services help persons enrolled in Permanent Supportive Housing (PSH) have better health outcomes, seven agencies in Suburban Cook County and Chicago will be collecting data on services provided to clients. The video below describes the project and its focus on three programmatic models for improving health - utilization of outpatient services, remaining in stable housing, and accessing public benefits. By using a shared set of defined health and human service terms for each outcome, we can learn more about the strengths of each service strategy on client health.
