  1. 2015 Housing Inventory Count & Point-In-Time Count Instructions/Definitions document
  2. A copy of this email text
  3. A spreadsheet with two tabs—one tab is a list of programs expected to be included in the final count and housing inventory; the other is a point-in-time count generated from HMIS for the night of Jan 28, 2015.  For HMIS users, this reflects the data as it currently exists in HMIS.  If this data is not correct, you will need to correct the data in HMIS so that your “0630 - SubCook Sheltered PIT 2014 – v7” report is accurate.
  4. A sample copy of the online 2015 Housing Inventory Count & Point-In-Time Count (forms A and B)

(do not submit these formsuse the link to complete the 2015 HIC & PIT online)

Non-HMIS Participants and SSVF/ESG Funded Rapid Re-Housing Providers

Non-HMIS and SSVF/ESG Rapid Re-housing providers need to complete both Forms A and B, of the online 2015 Housing Inventory Count & Point-In-Time Count.  Rapid Re-Housing providers should include only those persons housed in Suburban Cook County with rapid re-housing funds on the night of January 28, 2015.

HMIS Participants and CoC Funded Rapid Rehousing Providers

HMIS participants and CoC Funded Rapid Rehousing providers will need to complete only Form A of the online 2015 Housing Inventory Count & Point-In-Time Count, however submission will require an attached PDF copy of the ART report “0630-SubCook-Sheltered PIT 2015–v11” for the project.

In order to assemble all responses from the Suburban Cook County Continuum-of-Care, and make a timely submission to HUD, we would appreciate your submission as soon as possible, but no later than 5:00 pm, Friday, March 13, 2015.
