Here you can find Coordinated Entry report documents and documents necessary to complete all seven stages of the Phased Assessment. You should only use the Phased Assessment documents if you have been trained by the Coordinated Entry Team at Housing Forward. Download the training schedule and register for training at www.myentrypoint.org/training to receive that training.
Check out myentrypoint.org for all things Coordinated Entry including resources for people in need of housing assistance and for providers around Suburban Cook County.
Coordinated entry is a community-wide system that standardizes and expedites the process by which people experiencing homelessness or who are at imminent risk of homelessness access shelter, housing, and homeless resources. Coordinated entry will help suburban Cook County better target the limited resources provided by the homeless assistance system to people who are experiencing homelessness and need them the most.
